چکیده

-Opioid receptors (KOR) have been implicated in neuroprotection from ischemic neuronal injury, but less work has been performed with transient focal cerebral ischemia to determine the role of KOR during reperfusion. We tested the effects of a selective and specific KOR agonist, BRL 52537 hydrochloride [( )-1-(3,4-dichlorophenyl)acetyl-2-(1-pyrrolidinyl)methylpiperidine], and the standard KOR antagonist, nor-binaltorphimine dihydrochloride [nor-BNI; 17,17'-(dicyclopropylmethyl)6,6',7,7'-6,6'-imino-7,7'-binorphinan-3,4',14,14'-tetrol], on functional and histological outcome after transient focal ischemia in the rat. By use of the intraluminal filament technique, halothane-anesthetized adult male Wistar rats were subjected to 2 h of middle cerebral artery occlusion confirmed by laser Doppler flowmetry. In a blinded, randomized fashion, rats were treated with 1) saline (vehicle) 15 min before reperfusion followed by saline at reperfusion for 22 h, 2) saline 15 min before reperfusion followed by BRL 52537 (1 mg · kg 1 · h ) at reperfusion for 22 h, 3) saline 15 min before reperfusion followed by nor-BNI (1 mg · kg 1 · h ) at reperfusion for 22 h, or 4) nor-BNI (1 mg/kg) 15 min before reperfusion followed by BRL 52537 (1 mg · kg 1 · h ) and nor-BNI (1 mg · kg 1 · h ) at reperfusion for 22 h. Infarct volume (percentage of ipsilateral structure) analyzed at 4 days of reperfusion was significantly attenuated in saline/BRL 52537 rats (n 8; cortex, 10.2% 4.3%; caudoputamen [CP], 23.8% 6.7%) (mean sem) compared with saline/saline treatment (n 8; cortex, 28.6% 4.9%; CP, 53.3% 5.8%). Addition of the specific KOR antagonist nor-BNI to BRL 52537 completely prevented the neuroprotection (n 7; cortex, 28.6% 5.3%; CP, 40.9% 6.2%) conferred by BRL 52537. BRL 52537 did not produce postischemic hypothermia. These data demonstrate that KORs may provide a therapeutic target during early reperfusion after ischemic stroke.

Effect of 2 different anesthesia methods on stress response in neurosurgical patients with hypertension or normal

Hypertensive patients in neurosurgery are becoming more common, which increased the risk of surgical stress response. Meanwhile, the relationship between hypertension and anesthesia methods is unclear on the stress response. The purpose of this study is to compare the effect of different anesthesia methods on high-sensitivity C-reactive protein (Hs-CRP), blood glucose, and leucocyte levels in n...

Anesthesia for Neurosurgery in Infants and Children

1. Introduction Infants and children undergoing anesthesia for neurosurgical procedures present unique challenges for the anesthesiologist. Advances in neurosurgical techniques and technology have changed the face of pediatric neurosurgery and led to improved outcomes for pediatric neurosurgical patients. Basic concepts of neuroanesthesia care are similar in all ages. In this review, we will fo...

Central Sagittal Angle of the Sacrum as a New Risk Factor for Patients with Persistent Low Back Pain after Caesarean Section

Study Design Retrospective. Purpose This study investigated the possible association of persistent low back pain (LBP) with caesarean section (CS) under spinal anesthesia. Overview of Literature Many women suffer from LBP after CS, which is commonly performed under spinal anesthesia. However, this type of LBP is poorly understood, and there is poor consensus regarding increased risk after s...

Propofol Infusion Associated Metabolic Acidosis in Patients Undergoing Neurosurgical Anesthesia: A Retrospective Study

OBJECTIVE Propofol and volatile anesthesia have been associated with metabolic acidosis induced by increased lactate. This study was designed to evaluate changes in pH, base excess (BE), and lactate in response to different anesthetic agents and to characterize propofol infusion-associated lactic acidosis. METHODS The medical records of patients undergoing neurosurgical anesthesia between Jan...

Preoperative evaluation of the adult neurosurgical patient.

The purpose of this chapter is to provide the anesthesiologist with a focused review of the information needed to rationally approach the preoperative evaluation of the neurosurgical patient. The anesthesia preoperative assessment is a neuromedical evaluation of the patient's current condition integrated with the anesthesiologist's knowledge of the potential clinical and operative events that m...

Postoperative cognitive function following general versus regional anesthesia: a systematic review.

The effect of anesthetic technique on postoperative outcomes remains in question. This systematic review compares the role of regional versus general anesthesia, with a particular focus on postoperative cognitive function. Potentially relevant articles were identified by searching publicly available computerized databases for this systematic review. Any surgical procedure was accepted with the ...
